• REQUEST FOR INCOMPLETE GRADE: This form must be completed, as well as signed by the instructor and student before the end of the semester within which an extension is being requested. Incomplete grades are for special/extenuating circumstances only. Students are required to provide the Academic Office with appropriate documentation, verifying the need for an extension. Final approval for an extension is at the discretion of the Academic Office.
